Dandi offers several self-ID templates that are aligned with best practices for the US, UK, and Canada. You can select between basic and extended versions for each region based on your data collection needs. See our templates below, including their descriptions and use cases.

Self-ID US: EEO

  • Title: Self-ID US: EEO
  • Number of questions: 2
  • Regions supported: United States
  • Description: This is a template for companies looking to gather only the workforce data required by EEO law.
  • Use cases: Self-ID US: EEO allows companies to gather EEO data separately with distinct privacy settings, ensuring compliance with record-keeping requirements as defined by EEO law.

Self-ID US: Basic

  • Title: Self-ID US: Basic
  • Number of questions: 5
  • Regions supported: United States
  • Description: Self-ID US: Basic allows companies to collect workforce demographic information defined as standard—but not yet required—by the EEO: Gender, race/ethnicity, sexual orientation, veteran status, disability information.
  • Use cases: Self-ID US: Basic offers a simple, safe step up from mandatory EEO reporting. It’s great for companies unsure of what information to ask for or how much employees will be open to sharing

Self-ID US: Extended

  • Title: Self-ID US: Extended
  • Number of questions: 13
  • Regions supported: United States
  • Description: Self-ID US: Extended goes beyond mandatory and basic data collection, to capture a truly comprehensive set of workforce data.
  • Use cases: Self-ID US: Extended helps companies better understand the employee experience through the lens of identity. This template offers more inclusive and granular response values for fields like gender, race/ethnicity, and sexual orientation. Companies can also use Self-ID US: Extended to learn more about employees’ identities by introducing additional fields like caregiver status, education level, and family structure.

  • Description: Self-ID UK: Basic allows companies to collect workforce demographic information such as: Gender, race/ethnic group, sexual orientation, veteran status, and disability information.
  • Use cases: Self-ID UK: Basic offers a starting point for companies running their first Self-ID campaign. Self-IDUK: Basic is also great for companies unsure of what information to ask for or how much employees will be open to sharing.

Self-ID UK: Extended offers more inclusive and extensive response values for traditional fields like gender, race/ethnic group, sexual orientation, socio-economic background, nationality, or religious beliefs. Companies can also use Self-ID UK: Extended to learn more about employee identities by introducing additional fields like caregiver status, education level, and family structure.
